Fees and Insurance

Dr. Chouake is an out-of-network provider and does not participate in any insurance plans. Fees will be discussed prior to your first appointment. 

Although Dr. Chouake does not participate with any insurance plans, upon receipt of your direct payment, you will be provided with a paid invoice (superbill) that you may submit to your insurance carrier for reimbursement.  

Dr. Chouake has separate fees for an initial assessment, 45-minute follow up and 30 minute follow up sessions. 

You may contact your insurance company for details regarding your out-of-network benefits before starting treatment (including deductibles, out-of-pocket maximum, percent reimbursement), and regarding coverage of prescriptions and lab tests (often covered, but with varying out-of-pocket costs depending on the pharmacies and labs you use).   Your insurance company may ask you for billing codes to provide you more information about reimbursement rates.

 Billing codes most commonly use are:

* 90792 for the initial psychiatric evaluation/consultation

* 99213 + 90836 (or 99214 + 90836) for a 45-minute psychotherapy follow-up session

* 99213 + 90833 (or 99214 + 90833) for a 30-minute medication management follow-up session
